PARENT/GUARDIAN CONSENT, ASSUMPTION OF RISK, MEDICAL AUTHORIZATION, AND PARTICIPATION WAIVER

Northeast Wolves FC Youth Soccer Participation Agreement

Please review this Parent/Guardian Consent, Assumption of Risk, Medical Authorization, and Participation Waiver carefully before submitting the form below. We recommend printing or saving a copy of this page for your records before submission. By completing and submitting this form, you acknowledge that you have had the opportunity to review and keep a copy of this waiver.

This Parent/Guardian Consent, Assumption of Risk, Medical Authorization, and Participation Waiver is entered into by the undersigned parent or legal guardian on behalf of the minor participant identified in the registration form.

Participant and Program

I am the parent or legal guardian of the minor child identified in the registration form. I request that my child be permitted to participate in soccer practices, games, camps, clinics, scrimmages, tournaments, training sessions, travel, team activities, indoor and outdoor activities, and any related programs organized, sponsored, hosted, supervised, or attended by Philadelphia Soccerland Academy LLC/Northeast Wolves FC, its teams, coaches, volunteers, officers, directors, staff, contractors, facilities, field owners, leagues, sponsors, and affiliated organizations.

Acknowledgment of Soccer and Sports Risks

I understand that soccer and athletic participation involve inherent and unavoidable risks. These risks include, but are not limited to, falls, collisions with players or objects, contact with balls or equipment, uneven or slippery playing surfaces, weather exposure, dehydration, heat illness, overuse injuries, muscle strains, sprains, fractures, cuts, bruises, dental injuries, eye injuries, concussions, traumatic brain injury, sudden cardiac events, illness, infection, transportation-related risks, and other serious injury, disability, or death.

I understand that risks may arise from my child’s own actions or inactions, the actions or inactions of other participants, coaches, spectators, volunteers, or third parties, field or facility conditions, equipment, weather, or the ordinary risks of athletic competition and training.

Assumption of Risk

On behalf of myself, my household, and to the fullest extent permitted by law on behalf of my child, I knowingly and voluntarily accept and assume the risks associated with my child’s participation in Philadelphia Soccerland Academy LLC/Northeast Wolves FC activities. I understand that no program can eliminate all risk of injury or illness, even when reasonable safety rules, coaching, supervision, and facility precautions are used.

Release of Liability to the Fullest Extent Permitted by Law

In consideration of my child being allowed to participate, I, on behalf of myself, my heirs, family members, estate, representatives, and assigns, release and agree not to sue Philadelphia Soccerland Academy LLC/Northeast Wolves FC, its officers, directors, coaches, assistant coaches, trainers, volunteers, employees, contractors, representatives, sponsors, affiliates, leagues, facility owners, field owners, municipalities, school districts, property owners, and other related parties from claims arising out of my child’s participation, except to the extent such claims cannot legally be released.

This release is intended to apply to ordinary negligence to the fullest extent permitted by applicable law. This agreement is not intended to release claims that cannot legally be released, including claims arising from gross negligence, recklessness, willful misconduct, intentional misconduct, or other conduct that applicable law does not permit to be waived.

Parent/Guardian Personal Release and Indemnity

I understand that state law may limit or prohibit a parent from releasing a minor child’s own legal claims. Regardless, I personally release any claims that I may have in my own capacity arising from my child’s participation, including claims for loss of services, medical expenses paid by me, emotional distress, derivative claims, or similar parent/guardian claims, to the fullest extent permitted by law.

I agree to indemnify and hold harmless Philadelphia Soccerland Academy LLC/Northeast Wolves FC and the released parties from claims, costs, or expenses arising from inaccurate information I provide, my own conduct, my child’s violation of club rules, or claims brought by third parties caused by my or my child’s intentional, reckless, or rule-violating conduct, except to the extent prohibited by law.

Medical Fitness to Participate

I certify that my child is physically able to participate in soccer activities. I agree to disclose any medical conditions, allergies, medications, restrictions, prior injuries, concussion history, cardiac history, asthma, seizure history, or other health information that may affect my child’s safe participation. I understand that Philadelphia Soccerland Academy LLC/Northeast Wolves FC relies on the accuracy of the information I provide.

I agree that I am responsible for determining whether my child should obtain a medical examination before participation. If my child has any medical condition, recent injury, concussion, cardiac symptoms, or health concern, I agree to consult a qualified medical provider before allowing participation.

Emergency Medical Authorization

In the event of injury, illness, medical emergency, or suspected emergency, and if I cannot be reached promptly, I authorize Northeast Wolves FC coaches, staff, volunteers, representatives, athletic trainers, emergency medical personnel, physicians, hospitals, urgent care providers, dentists, or other licensed medical providers to evaluate, treat, transport, and obtain emergency medical care for my child.

I understand that Philadelphia Soccerland Academy LLC/Northeast Wolves FC does not guarantee the availability of medical personnel at any activity. I accept financial responsibility for medical treatment, emergency transport, ambulance services, medications, hospital care, follow-up care, and related costs not covered by insurance.

Concussion and Head Injury Acknowledgment

I understand that concussions and traumatic brain injuries can occur in soccer and may be serious. I agree to review concussion information provided by the club, league, state, school, or applicable governing body. I agree to notify Philadelphia Soccerland Academy LLC/Northeast Wolves FC immediately if my child has sustained a suspected concussion, head injury, or concussion-like symptoms.

I understand that if my child is suspected of sustaining a concussion or head injury during a Philadelphia Soccerland Academy LLC/Northeast Wolves FC activity, my child may be removed from participation and may not return until cleared in accordance with applicable law, league policy, club policy, or medical guidance.

Sudden Cardiac Arrest and Medical Warning Signs

I understand that sudden cardiac arrest and cardiac-related events, although rare, can occur during youth sports. I agree to review sudden cardiac arrest information provided by the club, league, state, school, or applicable governing body. I agree to notify Philadelphia Soccerland Academy LLC/Northeast Wolves FC if my child experiences chest pain, fainting, unexplained shortness of breath, seizure-like activity, dizziness, racing heartbeat, extreme fatigue, or any other concerning symptoms during or after exercise.

I understand that if such symptoms occur, my child may be removed from participation and may not return until medically cleared in accordance with applicable law, league policy, club policy, or medical guidance.

Illness, Communicable Disease, and Hygiene

I agree not to send my child to any Philadelphia Soccerland Academy LLC/Northeast Wolves FC activity if my child has fever, contagious illness, vomiting, diarrhea, unexplained rash, or any symptoms that may place other participants at risk. I understand that participation in group activities may involve exposure to contagious disease despite reasonable precautions.

Code of Conduct and Safety Rules

I agree that my child and I will follow Philadelphia Soccerland Academy LLC/Northeast Wolves FC rules, coach instructions, facility rules, league rules, sportsmanship expectations, and safety policies. I understand that failure to follow rules may result in removal from an activity, suspension, or dismissal from the program without refund, subject to club policy.

I agree that my child will wear required and appropriate equipment, including shin guards, proper footwear, and weather-appropriate clothing. I understand that jewelry, unsafe equipment, or unsafe conduct may result in restriction from participation.

Transportation

Unless separately agreed in writing, I understand that I am responsible for transporting my child to and from all Northeast Wolves FC activities. If transportation is arranged by the club, another parent, coach, volunteer, rideshare, bus, or third party, I understand that transportation carries additional risks and may require a separate authorization.

Photography, Video, and Media

I understand that Philadelphia Soccerland Academy LLC/Northeast Wolves FC may take photographs or videos during soccer activities for coaching, safety, team communication, website, social media, marketing, or promotional purposes. I may opt out of promotional use by notifying the club in writing. This opt-out does not apply to incidental group images, live events, league media, tournament media, or safety/coaching footage where removal may not be practical.

Privacy and Information Accuracy

I certify that all registration, emergency contact, medical, and guardian information I provide is accurate and current. I agree to update Philadelphia Soccerland Academy LLC/Northeast Wolves FC promptly if any information changes. I understand that the club may use submitted information for registration, team placement, emergency communication, safety, league compliance, payment, scheduling, and program administration.

No Guarantee of Outcome

I understand that Philadelphia Soccerland Academy LLC/Northeast Wolves FC does not guarantee athletic development, team placement, playing time, tournament selection, scholarship opportunities, professional opportunities, injury prevention, or any particular result from participation.

Severability

If any part of this agreement is found invalid or unenforceable, the remaining provisions shall remain in effect to the fullest extent permitted by law. The unenforceable provision shall be limited or modified only to the minimum extent necessary to make the agreement enforceable.

Governing Law

This agreement shall be governed by the laws of the state in which the activity primarily occurs, unless applicable law requires otherwise.
